TEG 6s Breakdown & Guidance
The TEG 6s system gets rid of K and Angle. Instead, it runs a Citrated Rapid TEG (CRT) to assess the overall MA. The fibin contribution is evaluated by running an additional TEG where the platelets are inhibited to get the Citrated Functional Fibrinogen (CFF). The platelet contribution to the CRT MA is the difference between the CFF and CRT curves.
Total MA ≈ CFF-MA + Platelet MA
- CFF (Functional Fibrinogen): An assay that inhibits platelets to measure the clot strength from fibrinogen alone. A low CFF-MA indicates hypofibrinogenemia.
Action: Treat with Cryoprecipitate or fibrinogen concentrate. - Platelet Contribution: The additional clot strength provided by platelets. This is calculated, not directly measured. A low platelet contribution with a normal CFF-MA points to thrombocytopenia or platelet dysfunction.
Action: Treat with Platelets.
